**Job overview**:
An exciting opportunity to join our newly formed Home First Rehab & Reablement team. Our service covers the whole of Salford facilitating clients discharge back into their own home. Our vision for Home First is ‘People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary’
**Due to the role working in the community a Full Driving Licence and the use of own vehicle is essential.**
**The Home First Reablement Service operates 7 days a week from 8am to 10pm and various shift working is available. The successful applicants will work on a rota that is provided in advance to cover shifts that includes weekends and Bank Holidays.**
**Main duties of the job**:
To provide support and person focused re-ablement as required to help ensure the efficient and effective running of the service.
To provide excellent customer service and establish and maintain communication between individuals, relatives, carers and the multidisciplinary team. To provide holistic patient care, correctly undertake tasks that have been delegated and to record information accurately and pass it on to the relevant people in the team in a timely manner. Duties may include:
- Assist the individual with personal care tasks, such as washing, dressing, eating, toileting and prompting medications
- Assist in the safe lifting, transferring, repositioning and movement of the individual
- To support individuals to live independently in their own homes and help achieve their own outcomes
- Observe, monitor and record clients physical and emotional well-being, and promptly report any changes to senior staff
- Record all visits and care on the Electronic Patients Record (EPR), Liquid Logic and in the Patients' personal folder
- Encourage clients to participate in Social and recreational activities

- NVQ Level 2 or Care Certificate (or equivalent experience - in either Therapy or Health Care)
**Knowledge, Skills and Experience**:

- Full Driving Licence is required and the use of own vehicle
- Good communication skills / the ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post
- Understanding of routine work procedures gained through a short induction period and on the job training
- Experience of working with members of the public or a desire to work in the Health Service
- Basic IT skills
