We are looking to recruit motivated practitioners with initiative and drive who are keen to contribute to the further development of our service. We work in closer collaboration with Children and Family Centre’s, Early Years, HENRY and Early Help as well as other agencies to improve outcomes and reducing health inequalities for children, young people and their families in our borough. For newly qualified practitioners, we have a preceptorship Programme in place to aid your development. We also offer a wide range of ‘in-house’ and other training opportunities for continuous professional development through our learning and development department as well as clinical supervision and mentorship. You will need to have the ability to work autonomously and as part of an integrated skill mix team. You need to have excellent communication skills and experience of working in partnership across organisational boundaries. You need to be willing to facilitate and support change in service delivery.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £3,898 to a maximum of £4,967).

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

1.To lead the delivery of the Healthy Child Programme, and ensure the 4, 5, 6 Service model is implemented in practice.
2. Ensure services are provided within the 4 tier model
- Your Community, Universal, Universal plus and Partnership plus. This will incorporate Building community capacity, and embedding this into caseload, team and locality management.
3. Responsible for the management of identified caseload, and classifying cases in line national stratify of Universal, Universal plus and Universal partnership plus.
4. Work in accordance to the Locality commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales, services provided are evidence based practice and the 6 High impact areas are embedded within practice.
5. To improve the health and wellbeing of children and young people especially those from disadvantaged groups.
6. To Identify, prioritise, develop and implement programmes of care to meet individual and local needs, taking into account cultural diversity. This will be achieved through evidence based assessments, effective care planning and implementation of a range of support packages for individual families, targeting support to families who require it.
