Dietetic Cancer Support Worker

We are looking for an enthusiastic individual with an interest in nutrition to work with our new upper GI cancer dietitian, existing cancer dietitians and MDTs in the role of a Support Worker.

This is a 2-year fixed term position working Monday to Friday.

Support patients through their diagnostic pathway.

The MSW will provide a link between nurse specialists and allied health professionals ensuring timely and accurate referrals are made to appropriate services.

Demonstrate excellent communication skills with patients and their families upholding the values of the organisation and Macmillan in delivering empathic. Kind and sensitive care

Assess communication needs of patients and their families to ensure they can understand the information being given

Ensure people who contact the service are aware that they are interacting with health professional who can access and signpost to a range of clinical services

Maintain appropriate and detailed documentation of patient contacts and interventions

Signpost and co-ordinate translation / alternative modes of communication as required

Professional communication with other staff encouraging the organisation’s values of teamwork, consistency and transparency

With the support of the Upper GI dietitian develop written information ,
education material and health and well being resources

Continued adherence to information governance policies and procedures and report and deviations

2. Support people to access appropriate information and support, signposting to internal and local services as appropriate
3. Look up, check or organise appointments and investigations to promote the timely progress for patients through their cancer journey
4. Complete referrals under the guidance of the dietitian as required
5. Provide the administrative support to ensure communications from the oncology dietitian are shared with the appropriate internal and external staff to promote patient care - with particular attention, but not limited to, holistic needs assessments and care plans and relaying of significant disease related information
6. Make pre-planned outgoing calls to offer support and assess the need for more specialist health professional intervention
7. Ensure all communications with patients, relatives and staff are accurately documented in a timely way - with particular reference, but not limited, to completing contact pages on The Somerset database
8. Assist in in self-supportive management at end of treatment.
