This is an advert for 2 nurse specialist roles who will work alongside each other to act as a key worker for survivors of cancer in the Thames Valley.
1. Paediatric CNS
2. Teenage and Young Adult CNS
Oxford is the Principal Treatment Centre for Children, Teenagers and Young Adults with Cancer who live in the Thames Valley. Approximately 160 patients in the Thames Valley region are diagnosed and treated for cancer each year with 130 survivors eligible for assessment within the Late Effects Service each year.
These post will work as part of the late effects core team, acting as a key worker. They will be able to accurately recognise, interpret, analyse, and make decisions in relation to patient risk and appropriately manage these patients with support of consultants within the LTFU/Late effects clinic
The post holder will assist in the development of services for long term follow up care for Children/Young Adult patients after the completion of cancer treatment and facilitate a smooth transition from the Children’s service to the Young Adult service. They will manage and co-ordinate follow up programmes working as part of the multidisciplinary team. They will continuously assess the needs of the service, identifying innovative approaches to facilitate development of individualised packages of care.
Banding will be agreed dependant on previous knowledge and experience in the field. CPD will be support within these roles if Band 7 requirements are not met.
Act as a keyworker for all eligible patients, signpost and liaise with other providers within the Thames Valley area to provide holistic care for patients
Complete holistic needs assessments for patients and formulate personalised care plans
Undertake pre assessment and nurse led clinics for Level 1 patients.
Undertake pre assessment and joint nursing and consultant clinics for Level 2 and 3 patients.
Utilise the Ready Steady Go programme for Paediatric patients.
Provide cross cover for the other CNS within the service

**Clinical Practice**:
**Senior Specialist Nurses will**:
Work at a highly specialised level, practicing autonomously as well as within a multi-disciplinary team and are self-directed.
Work clinically to optimise health and wellbeing, demonstrate autonomous decision making related to comprehensive history taking and physical assessment of patients, working within defined protocols and competency frameworks and scope of practice.
Work at a highly specialized level drawing on a diverse range of specialist knowledge in complex decision making to determine evidence-based therapeutic interventions, which may include advising the wider MDT.
Plan, prioritise and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use, providing direct support to patients and clients.
Act as key worker.
Use professional judgement in managing complex and unpredictable care events.
Draw upon an appropriate range of multi-agency and inter-professional resources in their practice.
Appropriately define boundaries in practice.
Promote a high standard of highly specialist nursing by initiating and coordinating the assessment, planning, delivery, and evaluation of the holistic needs of patients/families through evidence-based practice following agreed policies, protocols, and guidelines.
Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities, and agencies. Utilise evidence-based information through a variety of communication media.
Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity/data collection as required.
