Long Term Condition Care Co-ordinator

Care Co-ordinator roles are new to Primary Care and we are looking to recruit a Long Term Condition Care Co-ordinators within the Tame Valley Primary Care Network (PCN) across the borough. This role will involve maintain an overview of clinical achievement and ensure the efficient and timely recall of patients with long term conditions for clinical review.; supporting the PCN manager and associated practices by co-ordinating activity and providing an efficient, well organised administrative and operational support to the clinicians and managers in the network to ensure effective timely delivery of the PCN objectives. Main Roles & Responsibilities: To be familiar with the features and functionality of the EMIS Web system and any associated programmes to ensure knowledge is current and systems are efficiently and effectively utilised. Liaise with key contacts to ensure all practices and work processes are effectively contributing to clinical performance and meet contractual clinical requirements across the network To be familiar with, and maintain up to date knowledge of the GP and PCN Enhanced Service Contract requirements (QOF, Vaccinations and Immunisations, IIF etc.) Keep abreast of the SNOMED changes in Technical Requirements Specifications to meet requirements of the GP and PCN Enhanced Service Contracts Maintain an overview of Population Manager achievements to ensure the Practices are on track for clinical performance Proactively manage and liaise with key contacts to address any areas of potential or actual under performance Within defined protocols enter and amend clinical SNOMED codes in patients medical records Assess and standardise current recall systems across the Network and implement the most efficient process To conduct searches on the clinical system to identify patients for recall and ensure invites are made in a timely manner via telephone, text or letter and appropriately code in the patients medical record Operate a programme of recall to harmonise recalls for patients on multiple registers, thereby reducing the impact on patients and reduced unnecessary patient contacts Understand the monitoring requirements of patients with long term conditions Understand the monitoring requirements of high-risk drugs, implementing a standard recall process across all practices Operate the programme of recall to smooth workload across the financial year as much as possible Ensure patients on recall registers are booked for review in accordance with the chronic disease booking protocol Compose appropriate patient letters and information leaflets.
In circumstances where it is known that a member of staff has communicated information to an unauthorised person, those staff will be liable to dismissal. Moreover, the Data Protection Act 2018 also renders an individual liable to prosecution in the event of unauthorised disclosure of information Health and Safety All employees must be aware of the responsibilities placed on them under the Health and Safety at Work Act 1974 to ensure that the agreed safety procedures are carried out to maintain a safe working environment for patients, visitors and employees Review and ensure systems are in place for the proactive management of risk registers Infection Control All employees of Viaduct Care are required to contribute to the management of infection control and be proactive in awareness raising and prevention Ensure that all guidelines, protocols and procedures undertaken within the service are in line with evidence based practice for infection control Safeguarding Children and Adults All staff must act in such a way that at all times safeguards and promotes the health and well-being of children and vulnerable adults. Familiarisation with and adherence to the policies and procedures of the Local Safeguarding Boards (Children and Adults) and those of Viaduct Care is an essential requirement of all staff as is participation in mandatory safeguarding training in accordance with individual roles and responsibilities. Environmental impact should be reduced by recycling where possible, switching off lights, computers, monitors and equipment when not in use, minimising water usage and reporting faults promptly.
