We have an exciting 14 month fixed term opportunity for a Patient Care Navigator with the Alcohol care team. This opportunity is help improve the service and offer admin support to the current CNS team. Applications from band 2 nursing assistants or other AHP welcome.

The Alcohol care service is a new service that is offering support and guidance to inpatients in Grimsby and ensuring safe discharges and referrals are being made.

Main duties consist of

1. Provide administrative support to the department to support the safe discharge and cquin data recorded.
2. Review of patients to support the medical and nursing teams in discharge planning.
3. Assis in facilitating MDT meeting and liaising with medical, nursing, therapy and social teams.
4. Liaise with other departments to ensure timely investigations are in place and available.
5. Escalate barriers to timely diagnostics, assessments and reviews to the appropriate team.
6. Responsible for keeping the Web V electronic system up-to-date, looking at new referrals and booking in fibro scans.
7. Support with queries from patients, relatives and other departments.
8. Review diagnostic test requests (e.g. blood tests, x-rays) ensuring results are brought to the
attention of the relevant Medical or Nursing staff in a timely manner and are acted on.
9. Act as a liaison point and provide information for external agencies as required (e.g. GPs, community providers, care homes and social care agencies).
10. Attend meetings as required in relation to service developments and act as admin support, be able to take notes.
11. Referral to Occupational, physiotherapy and safeguarding teams as appropriate.
12.Excellent IT skills and Knowledge

13. Clerical support.

14. Offer education and training to the wards.
