Monitor the uptake of the Annual health checks at the GP practices from the data received from NHS England. Provide support and specialist advice to those practices with low uptake. To assist GP practices with the development Health Action Planning. To support the development of Learning Disability registers, flagging systems and recognition of learning disability by Health Services.

Develop own knowledge in health inequalities of people with Learning Disabilities and local health issues and share this knowledge with Primary care and act as a resource for staff. Adapt local training packages to reflect current research and national drivers. To advise and support healthcare services to make reasonable adjustments for their learning disabled population. Provide specialist advice and support to other health and social care professionals in line with best evidence based practice.

To advise organisations on healthcare needs of adults with learning disabilities, To provide Learning Disability health care needs training to a variety of services. To engage with individuals, families and carers in consultation exercises and events relating to healthcare and wellbeing. Promote equal access and benefits from mainstream health & wellbeing services. To raise awareness of the health needs of people with learning disabilities by identifying and meeting training needs through both the delivery of training and by signposting carers and workers towards appropriate services.

Lead local improvements regarding health information for people with a learning disability and their families. This may include developing and enhancing local pathways through services. To provide specialist advice relating to people with learning disabilities, across specialist services, primary & secondary care. To complete and participate in relevant research.

To undertake regular clinical and other audits of Health Facilitation work, this may include satisfaction surveys. Participate in audit programmes and data collection appropriate to the role; eg ICB, ICS, NHS England, NDTI. Contribute to the planning and implementation of nationally driven programmes such as STOMP and LeDeR. To ensure the development of health promotion, health action planning and health facilitation is developed in line with national and local requirements.

To interpret national recommendations related to the healthcare needs of people with learning disability into local action plans. Be involved in developing policies and strategies at a local level and influence service developments. Advocate and support patients with learning disabilities inclusion within local health related developments and initiatives. Support primary care through complex issues and advise on appropriate evidence based care management.

Disseminate good practice as part of leadership role to relevant teams and services Challenge any emerging issues and institutional discrimination in services, shared with the team by people with learning disabilities and their carers. Active involvement in compilation of comprehensive data regarding the Learning disabled populations health & social care needs. Participate in formal review and evaluation of the Health Facilitation Service and contribute to the development and implementation of the Learning Disability Health Facilitation role. To develop the ability to provide mentorship and guidance to student nurses and other learners.

Participate and adhere to quality initiatives within the organization. Work autonomously to prioritise and organize own workload and diary Retain membership of regional and national nursing networks to identify and share knowledge of best practice. Build relationships and networks with carers, people with learning disabilities and services locally in order to support changes and consultations. Act as a conduit for information sharing across local specialist learning disability services and the NHS.

Promote and develop information in a variety of formats to enable people with differing communication abilities to access services. Manage the stock control of health promotion leaflets and easy read resources based on the needs of the GP surgeries. Manage the availability of leaflets and resources for health promotion/screening programmes e.g. flu.
