Support Coordinator

We are looking for an innovative, passionate and professional individual with excellent interpersonal and organisational skills to join the Stroke Recovery Service based in Mid/South Cheshire, in this home based role.

Position: S1072 Stroke Support Coordinator

Location: Homebased, Mid/South Cheshire (Crewe/Vale Royal) however, extensive travel will be required as part of this role

Hours: Part-time, 28 hours per week

**Salary**: Circa £20,470 per annum (FTE circa £25,500.00 per annum)

Contract: This is a fixed term contract until 31 March 2024. Our services are contracted, we currently have funding for this contract until 31 March 2024.

The Role

Reporting to the Service Delivery Coach, you will have the ability to support Stroke Survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.

Key responsibilities will include:
*Supporting new stroke survivors and their carers from hospital discharge into the community.
*Providing personalised information, advice and support.
*Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
*Working with other health and social care professionals across the stroke pathway to ensure high quality support.

About You

You will have:
*Experience in a caring profession, ideally supporting people with disabilities.
*Excellent IT skills and an ability to maintain accurate records.
*An affinity with the values of the Stroke Association.
*A flexible approach and an ability to effectively manage a caseload.

To fulfil the role you must be resident in the UK and have the right to work in the UK
