The Response Hub is a newly developed 24 hour service providing support to service users in the HMR area.

We currently have a vacancy for a Band 6 practitioner to join our Response Hub team in Rochdale. The successful applicant will work within the main hospital of Rochdale Infirmary, providing assessments and care for adults who require support out of hours.

**Key Requirements of the Person Specification**:

Work as part of the Response Hub team to deliver an effective and efficient
response to people who access the service, providing clinical assessments and interventions focusing on relapse prevention and improved quality of life for service users.
To undertake mental health assessments for people in need outside of working hours.
Offering highly developed clinical and risk assessments, risk formulation and management plans.
Provide support and training to non-mental health professionals regarding
recognising mental health conditions and appropriate management and onward referral pathways.
Assess service users with a range of complex physical and mental health needs.
Assist in the gate keeping process.
To effectively discharge patients and signpost to alternative services.
