Morgan Hunt are currently working with a housing provider whose mission is to transform the lives of vulnerable people. This is done through accommodation based services as well as community outreach.
Our client are looking to recruit an Outreach Worker, covering Sheffield on a permanent basis. As an Outreach Worker, you will manage your own caseload of young people and support them towards living independently to allow them to sustain their tenancy.

**The role - Outreach Worker**
- Providing ongoing assessments of service users needs
- Conducting welcome and induction meeting with service users to establish their needs and to ensure the service user understands their tenancy agreement
- Arranging appointments with various partner agencies and liaising with designated Social Worker around progress and individual needs of service users
- Supporting service users to develop their independent daily living skills through things such as budgeting, healthy eating, healthy relationships and accessing health services
- Accompanying service users to participate in activities of interest within the community
- Preparation of reports and keeping accurate record around the progress of clients
- Assisting with move on of service users when required
- Experience of working within a similar role providing floating support to individuals with complex needs
- Experience of supporting young people to sustain a tenancy
- Experience of working independently and managing a caseload of service users
- Experience of multi agency working with other professionals
- A driver is essential for this role due to the travel required
- An Enhanced DBS is essential for this role
