Berkshire Healthcare NHS Foundation Trust is currently seeking a Mental Health Practitioner (CPN120) to join their Memory Clinic based in Maidenhead. This role will be full time Monday to Friday 9am - 5pm for the next 12 weeks.

In this role you will be located in and working across an area with all the benefits of country living with the convenience of big city life.

**What you will be responsible for**
- Undertake initial comprehensive assessments in a range of settings including patient's own home, considering the person's physical, psychological, social and spiritual needs.
- Ensure that all programmes of nursing care interventions are based upon the service user's individual needs and that they are consistent with the Memory Clinic Care Pathway
- Identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
- Participate and support on the MSNAP accreditation process
- Provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community such as Adult Social Care Service, Residential and Nursing Care Homes, Sheltered Housing, Day Centres etc.

The role will involve, but not limited to you performing the above.

**You will have the following qualification/experience**:

- A minimum of 6 months paid experience as a Mental Health Practitioner for a UK hospital environment within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.

**Depending on the Trust, you may require variable additional certification and skills.**
