Job title - Trusted Assessor Discharge Partner

Contract/Hours - Fixed term for 12 months, Full Time 37.5 hours per week, Monday - Friday 08:30 - 16:30

Location - Site-based role at NBT/UHBW (BRI)

Salary - £35K - £42K (Band 6)

Deadline - 5pm on 30th June

**+ Role Purpose**

The purpose of the role is to facilitate and support effective and timely discharges from Acute Hospital settings into Care Homes, appropriate discharge pathways or returning home with packages of care. The role will aim to make the very best use of our collective system resources, facilitating smoother, quicker hospital discharges, and reduce post-admission problems in community settings and readmissions to hospitals.

The focus is to achieve positive outcomes for people using our Health and Social Care services in the Bristol locality.The primary responsibility of the Trusted Assessor Discharge Partner is to act as an unbiased assessor on behalf of care providers in relation to undertaking professional and timely patient assessments.

The postholder will be required to achieve this by establishing and maintaining trusting relationships with both care providers, acute trusts, community health provider and the wider system whilst remaining independent The Trusted Assessor Discharge Partner role is a new position funded by Care and Support West. This is an exciting opportunity for a motivated and enthusiastic individual to work as an ambassador within the discharge service piloting the role with a view to demonstrating its value in the Health and Social Care system in the Bristol locality.**+**

**Qualifications**

**Essential** - Registered Health / Social Care Professional, including Nurse, or Allied Health Professional with appropriate current registration and evidence of continued professional development

**Desirable**
- Diploma in Management - Level 5 CMI or equivalent leadership qualification, registered Managers Award - City & Guilds, Knowledge re. complexities of caring for Older People or similar university-accredited qualifications re. frailty

**+ Essential Job Functions**
- To be the link worker in the acute trust representing care providers
- To be a link for patients and their families in the acute trust who can provide expert advice and assistance regarding community-based care packages and care homes in the Bristol locality
- To build effective relationships with multi-disciplinary colleagues in the acute trust, Bristol City Council, BNSSG ICB and the care provider market in the Bristol locality
- To be conversant with the discharge process within the acute trust
- Being conversant with Transfer of Care Form and other relevant documentation in the acute trusts
- To work to facilitate timely discharges into the community for Bristol patients on pathways 0 - 3
- To identify any patients with established packages of care or care home placements that can return with these in place within 48 hours of being deemed as medically optimised for discharge
- To complete full telephone assessments with providers for patients who have been navigated through to a particular provider by the Transfer of Care Hub
- To identify where there are barriers to discharges from the acute trust to providers and work with them to facilitate successful discharge
- To have an understanding of the contract arrangements for pathway 2 and 3 bedded facilities e.g. time frames for the screening of referrals
- To work with acute colleagues to ensure that safe discharges are facilitated with all essential medication, equipment etc. in place
- To maintain, monitor and provide set KPI’s on discharge flow in line, reporting these to key partners formally on a regular basis
- To be an ambassador for the Trusted Assessor Discharge Partner role, always acting in a calm and professional manner
- To look to see how things can be done differently - working in a proactive manner and constantly investigating new initiatives and ways of working
- Where there are barriers to discharge or challenges either within the acute trust or with providers, there is a clear understanding of how to escalate/report these

**Essential**:

- Excellent interpersonal skills and communication skills, both written and verbal.
- Ability to influence across multiple professions and organisations
- Ability to build and sustain effective relationships with both senior staff and organisations.
- Experience of working in or with a care providers, either in a care home, reablement centre or domiciliary care setting.
- Knowledge of the Health & Social Care environment
- Self-starter, able to work autonomously within a complex environment
- Excellent organisational skills
- An ability to problem-solve and decision making
- Ability to collate, interpret and present data
- Experience in assessing service-user needs
