**Job Title: Integrated Care Coordinator/Administrator**

**Accountable to: Hoveton & Wroxham Medical Centre & North Norfolk Primary Care**

**Place of work: Hoveton & Wroxham Medical Centre, a practice in NN4, (a PCN in North Norfolk comprising of Acle Medical Partnership, Brundall Medical Partnership, Blofield Surgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham & Stalham Green Surgery)**

**Principle Liaisons: Administration Manager, Hoveton & Wroxham Medical Centre & North Norfolk Primary Care, Primary Care Network Clinical Directors, North Norfolk Primary Care & North Norfolk GP Practices.**

**Salary Range**: **£20,628 per annum**

**Contract type: Full time (37.5 hrs/week)/Negotiable**

**Job Purpose**:
The post holder is a Care Coordinator, who will receive full training and guidance to support the coordination of care through effective administration activities based in Hoveton & Wroxham Medical Centre, within the Primary Care Network of NN4. The role is administrative covering the following areas of work:
**Key Responsibilities**:
Training will be given to meet the responsibilities which include but are not limited to the following:

- To work within Hoveton & Wroxham Medical Centre to support personalised care, including the administration of the integrated care of patients.
- Summarising patient care records within SystmOne clinical IT system.
- Assist with the management of Hoveton & Wroxham Medical Centre operations.
- Developing competences to enable clinical system data management and reporting.
- Processing correspondence and associated information management to meet clinical quality requirements.
- support patients to utilise decision aids in preparation for a shared decision-making conversation
- help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
- Any other delegated duties appropriate to the post.
- Attend and support practice meetings.

- A commitment to maintain a high professional standard of care and keep up to date with all guidance relevant to the post.
- A commitment to ensure all Health and Safety requirements and Infection control measures are met and to report any problems to the practice manager.
- A commitment to the effective use of practice and NHS resources.
- An awareness of own limitations and experience.
- Cooperate with appraisal meetings
- A commitment to be enrolled in, undertaking or qualified from appropriate training as set out by the Personalised Care Institute
- To assist the practice, working closely and in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider and Health and Wellbeing Coach(es)
