The Trainee ACP will be a senior member of the leadership team and responsible for the delivery of safe and effective nursing practise across the community mental health transformation teams working to bridge the gap across primary and secondary care mental health. The Trainee ACP will ensure clinical systems are in place to support and monitor practice and performance and support the team manager in the operational delivery of the service. The Trainee ACP will provide direct clinical care and act as a role model for exemplary nursing practise in the PCNs and community mental health teams. Working with other Trainee ACPs the post holder will provide trust wide leadership and support within their own discipline.

**KEY RESULT AREAS**: 1. Clinical Practice 1.1 Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. of history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments). 1.2 Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

1.3 Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, life style advice and care. 1.4 Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers. 1.5 Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings. 1.6 Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.

1.7 Work as an independent non-medical prescriber within areas as required and designated, taking responsibility for the clinical assessment of the patient, establishing a diagnosis anda plan for that patients care and treatment. 2. Leadership and Management 2.1 Pro-actively initiate and develop effective relationships, fostering clarity of roles within the team, to encourage productive working. 2.2 Ensure that there is a clearly defined model of care and that all staff understand and can articulate how the model affects their practice.

2.6 Be responsible for ensuring an effective programme of induction for all new personnel including bank and agency and ancillary staff working within the defined clinical area is delivered. 2.7 Ensure all newly appointed Qualified Nurses/ AHPs and Nursing Associates /Practitioners receive Preceptorship to a high standard and within designated timescales. 2.8 Ensure all staff receive effective management and clinical supervision in accordance with Trust and locally agreed standards. 2.9 Maintain a high standard of safety, safeguarding the welfare of patients and staff in accordance with Trust requirements and Health and Safety legislation.

2.10 Be involved in the recruitment and selection, deployment and supervision of staff within area of responsibility. Ensure that appropriate staffing levels and skill mix are available to meet the changing needs of the service. 2.11 Ensure safe practice in relation to the administration of medicines, complying with Trust policies, procedures, locally agreed standards and NMC Code of Conduct 3. Education 3.1 Facilitate collaboration of the wider team and support peer review processes to identify individual and team learning.

4.2 Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others. 4.3 Be responsible for all aspects of nursing/clinical audit within the defined area of responsibility acting as Audit Champion within the respective area of practice. 5. Professional 5.1 Supervise and co-ordinate the professional work of nursing staff, complying with the NMC Code of Professional Conduct.

5.2 Ensure the provision of planned individualised care, complying with Trust policies, procedures, locally agreed standards and NMC guidelines. 5.3 Ensure the provision of a robust Named Nurse framework of care delivery in keeping with locally agreed standards. 5.4 Be the primary contact for clinical issues including the coordination and facilitation of responses to concerns, incident reports and complaints and requirements for performance information when requested by the operational team manager 5.5 Ensure a timely response to requests for reports or information from line manager and others. Provide information of a high standard.
