The post holder will work as a member of the Trinity multi
- disciplinary team, liaising closely with therapy and nursing staff to design person centred activities contributing positively to the emotional and psychological well being of patients and carers. Empowering patients and their carers to explore new opportunities and new interests whilst facilitating existing skills. Planning and organisational skills: Work with members of the teams to prepare and implement a varied programme including social, leisure and recreational activities with patients/carers. Organise and facilitate group activities to promote social interaction.

Arrange individual activities for patients as appropriate. Create a timetable of general activities for the inpatient unit on an individual and group basis dependant on needs. Participate in the triage of referrals to the service. Support patients/carers during therapeutic activities, either individually or with groups of patients.

Work with teams to ensure patient fundamental care needs are met and dignity is maintained. Actively plan fundraising events. General duties: Network with appropriate partners and agencies to promote healthy relationships between Trinity, community, and Acute Trust. Actively publicise activities and monthly campaigns to ensure the profile of events is heightened.

Work alongside colleagues (ACP Physiotherapist, Occupational Therapist, and nursing teams, etc) to support the delivery of planned groups. Work alongside volunteers to support and coordinate their work. Care responsibilities: Recognise and report changes in the patients condition and any issues in respect of their health and management of care to the lead clinician for the day. Ensure an understanding of caring for and the safeguarding of vulnerable people.

Minimise the risk of infection in accordance with Trinity guidelines. Ensure and demonstrate written and electronic communication and documentation of clinical records. Encourage patients to maximise their independence. Engage in supervision group and/or individual.

Policy and Service Development Work as a member of the Living Well Service Team as this develops to ensure high quality service delivery in an ethical and effective manner. Carry out the role in accordance with current legislation, the Trinity values, policies and procedures. Support, develop and facilitate relevant group work either within Living Well Service, IPU or the bereavement support services. Effort and Environment Physical Physical effort may be required on occasions (moving and handling patients).

Mental Frequently required to concentrate for one or two hours at a time. The work pattern may be unpredictable. Emotional Frequent exposure to distressing or emotional circumstances encompassing a main part of the role with occasional exposure to highly distressing circumstances. Recognise difficulties in relation to dealing with situations within palliative care in self and other colleagues and to engage and encourage other colleagues to participate in supervision for support.

Employees will familiarise themselves with the Health and Safety policy and procedure as well as the fire procedures and ensure a safe working environment for self and others in line with these. Safeguarding All employees should make themselves aware of the policies and procedures of safeguarding, take personal responsibility as far as is reasonably practicable to safeguard children and vulnerable adults, complete statutory and mandatory training and take appropriate action as set in the Hospices policies and procedures. Infection Control All employees have a responsibility to ensure the risk of infection to themselves, colleagues, patients relatives and visitors is minimised by: Attending mandatory training and role specific infection prevention education and training. Challenging poor infection prevention and control practices.
