**Clinical Le**ad **Anticipatory Care Planning**

We would like to invite applicants for the post of Clinical Lead Advanced Clinical Practice in Lanarkshire. This post will aims to support the North and South Lanarkshire MD teams in the provision of a high quality information, scoping and leadership around anticipatory care planning from the perspective of both the whole system and the lens of general practice.

Please note this is a fixed term post for 6 months.
