The post holder will be responsible for facilitating service user activity provision and safe discharge from the service back to the community. This includes recreational, social and educational group and individual activities to service users who are resident on the ward. The post holder will also support with facilitating service user access to community activities outside of the unit in order to meet identified goals. The post holder will work in close collaboration with other professionals, including the wider Multi-Disciplinary Team (Nursing, Psychiatry, Psychology, Allied Health Professionals and Arts Therapies) in implementing care plans that promote service user empowerment and provide high quality, individualised care, which addresses the needs of the service users and meets individual health needs. The post holder will also support with the safe and effective discharge of service users from the inpatient service to the community.- Lead, plan, promote, deliver and evaluate recreational, social and educational activities in accordance with the service user’s needs to meet goals identified by the MDT.
- Support service users to access activities both within and outside of the unit that support their interests and recovery.
- Ensure all service users are aware of activities which may be of benefit to them and how they can be accessed.
- Initiate, develop and implement new activities in response to service users’ needs.
- Actively promote the benefit of undertaking therapeutic activity within the inpatient setting to service users and other team members.
- Contribute to the monitoring of the physical environment ensuring that it is safe and conducive to the therapeutic work of the unit, including ensuring appropriate materials are prepared for use in activities, monitoring potential risks.
- Contribute towards the ongoing quality of service and care and identify any risk issues in within an agreed and monitored supervisory framework.
- Provide support to service users in all elements of their daily living where appropriate.
- Carry out practical tasks, identified as part of the service user’s care plan alongside completion of supportive observations.
- Provide support to the service user and the MDT during the transition and discharge process to facilitate successful discharge of the person to the community.
