**Responsibilities**:

- Provide support to individuals with daily living activities such as meal preparation, personal hygiene, and medication management.
- Work collaboratively with other healthcare professionals to develop and implement care plans that prioritize individual safety and wellbeing.
- Identify and assess potential risks to individuals and take appropriate action to safeguard their welfare.
- Monitor and document individuals progress, and report any changes or concerns to the management.
- Provide emotional and social support to individuals and their families, while also maintaining professional boundaries and ensuring individual confidentiality.
- Assist indviduals with accessing community resources and services, taking into account their individual needs and preferences.
- Maintain accurate and up-to-date individual records, including safeguarding documentation.
- Attend team meetings and participate in ongoing training and professional development, with a focus on safeguarding.

Qualifications:

- No experience needed, full training provided.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team, while also prioritizing individual safety and wellbeing.
- Valid driver's license and access to a reliable vehicle.
- Shifts consist of 7am - 2.30pm, 3.30pm - 10.15pm on a rotation basis and every other weekend

**Job Type**: Permanent

**Salary**: £11.25-£14.00 per hour
