**About The Role**:
We are recruiting three full-time Assessment Support Coordinators to work within the following teams:

- **Dorchester Locality Team**:

- **Weymouth Locality Team**:

- **Peripatetic Team **(based flexibly in various teams across Dorset)

As an Assessment Support Coordinator, you will be working alongside Social Workers, Occupational Therapists, and health colleagues across a range of settings to support people to remain as independent as possible and linked in with their local networks.
- you will be undertaking strengths-based assessments and support planning, in accordance with the Care Act 2014
- you will assist individuals to identify their goals and how best to achieve them to support their independence, and where appropriate complete mental capacity assessments
- you will be working as part of a busy team and alongside other teams within the local authority. Multi-disciplinary practice will be integral to the role

**Further Information**:
This is an exciting time within Adult Social Care as you will be involved in transforming the way we work and have an integral role in positively implementing change to meet both organisational and customer demand.

This is a full time role at 37 hours per week.

This post requires significant travel. You must have a vehicle (or transport we deem suitable) available for use as needed.

**Interviews will be held via Teams and will take place between Wednesday 26th and Friday 28th June.**
