**Job summary**

We are looking to recruit a Care Co-ordinator in the Manor area of Sheffield. Ideally this will be a full-time position, but we are happy to consider part-time and job share

We are looking for an enthusiastic, motivated person to join our friendly but busy team. You will need to be able to integrate with our multi-disciplinary team to deliver effective co-ordinated care to patients. Applicants should ideally have experience of working in a medical setting and be aware of the different healthcare professional roles within the practice.

Along-side other members of the GPA1 PCN, you will work towards tackling health inequalities in health and social care settings.

**Main duties of the job**
- Provide coordinated care and navigation for patients and their carers across health and care services, working closely with social prescribers, health and wellbeing coaches and other primary care professionals.
- Proactively identify and work with a cohort of patients to deliver personalised care.
- Help people to manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care.
- As part of the multidisciplinary team, build relationships with staff within the PCN, attending relevant meetings, providing information and feedback to meet the requirements of the network DES.
- Be proactive in developing strong link with local community services, and in encouraging equality and inclusion.
- Liaise directly with care homes to ensure all patients have a personalised care plan.
- Provide support with the delivery of Covid and Flu vaccination programmes.
- Manage and prioritise own workload.

**About us**

The White House Surgery is an established training practice, based in Manor, Sheffield. We are members of GPA1 Primary Care Network along with 5 other surgeries.

We have a large multidisciplinary team which includes GP's, ANP's, Nurses, HCA's and a Pharmacist. This team also extends to other health care professionals including junior doctors, paramedics, physiotherapists and mental health workers. Our busy administration team and receptionists provide a friendly and professional service to our patients.

The White House Surgery prides itself on teamwork and teaching, whilst also providing a quality service to an area with a number of social challenges.

**Job description**

**Job responsibilities**
- Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribers, health and wellbeing coaches and other primary care professionals.
- Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care
- GP's and other primary care health professionals to identify and manage a caseload of patients, making onward referrals where needed
- Support patients to be prepared to have shared-decision making conversations.
- Help people to manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care
- Holistically bring together all of a persons identified care and support plan, in line with best practice, based on what matters to the person.
- Support people to take up training and employment, and to access appropriate benefits, education courses, peer support, and/or personal health budgets where applicable.
- Help patients manage their needs through signposting to other relevant services.
- As part of the multidisciplinary team, build relationships with staff within the PCN, attending relevant meetings, providing information and feedback.
- Be proactive in developing strong link with local agencies, and in encouraging equality and inclusions
- Liaise directly with care homes and other key providers, and compile and circulate relevant information across stakeholder groups
- Understand, put in place and adhere to safeguarding protocols for vulnerable individuals
- Capture key information to enable comprehensive and accurate records of support, inputting these into clinical systems as required and adhering to data protection legislation

**Qualifications**

**Essential**
- GCSE grade A-C in English and Maths

**Experience**

**Essential**
- Experience of working within health/community/social care or voluntary setting
- Computer and software experience inc, word excel and powerpoint

**Desirable**
- Knowledge of SystmOne
