As an Associate Practitioner in the Wakefield Community Rehabilitation Team you will carry out interventions with service users within their own property and within community settings enabling people to live an independent a life as possible. Service users identify goals with their Occupational therapist and as each individual is unique the intervention you will be providing will be diverse, leading to each of your working days being different. You will work in an agile way, planning your visits in a way which works for you and the service. Although within each day a lot of your work you will be a lone worker we work closely as a team, with excellent communication skills and team members feel supported within their roles. The team also holds strong links with wider community mental health teams.

The team work to a recovery model of care, enabling a persons independence.

Within the role of an associate practitioner within the team you will support service users to develop their skills, decreasing this intervention as they become more independent, also at times increasing your input due to an assessed increase of need. A persons skills may include domestic tasks, gaining confidence, managing anxiety/ stress, learning positive coping strategies, accessing the community, or developing interests. You will liaise and involve carers and family members within a service users care when needed.
