University Hospitals Bristol & Weston NHS Foundation Trust (working in partnership with North Bristol NHS Trust) has been the designated Principal Treatment Centre (PTC) for Teenagers and Young Adults (TYA) with cancer in the south west of England since 2011. The PTC oversees the delivery of specialist malignant Haematology and Oncology care along with the provision of emotional, social and practical support to young people diagnosed with cancer between 16 and 24 years.

Job purpose
1. To coordinate and keep records for the weekly TYA Multi-Disciplinary advisory Team (MDaT) meeting and clinical meetings.
2. To lead on specific projects/tasks as agreed on behalf of the TYA Cancer team
3. To be able to act appropriately in a professional manner and be able to communicate sensitive / confidential information with staff, patients and relatives both at UHBW and across the TYA Cancer Network in the south west.
4. To record outcomes for TYA MDaT and update the Somerset Cancer Register.
5. Provide administrative support to the TYA Lead Clinician and TYA Nurse Consultant, in particular to ensure that meetings are organised and actioned effectively at the UHBW; this includes issuing agendas and papers & taking minutes for the monthly TYA cancer business meetings and TYA cancer team meetings

The post holder will be responsible for the organisation and tracking of TYA cancer patients across all departments to which they may traverse These patients may need to be identified from other sources and tracked (including at palliative care MDT meetings). The post holder will also provide day-to-day intelligence about progress against Patient Tracking List (PTL) plans, troubleshooting blockages in patient pathways and providing proactive information in anticipation of potential problems, escalating as necessary.

The post holder is responsible for ensuring accurate information is electronically stored in a timely and co-ordinated manner. They will become an expert user of the Somerset Cancer Register and the bespoke TYA Integrated management system (TYA IMMS); this will involve providing training and support to other users.

The post holder will work unsupervised for the majority of the time but will be supported as necessary by core members of the TYA cancer team. The post holder should have the ability to work in a team or independently.
The post holder will provide administrative support to the TYA Lead Clinician and TYA Lead Nurse, in particular to ensure that meetings are organised and actioned effectively at the UHBW; this includes issuing
agendas and papers & taking minutes for the monthly TYA cancer business meetings and TYA cancer team meeting.

The post holder will have a pivotal role in enabling the Trust and TYA cancer team to deliver a high standard of care and communication throughout the pathway. Effective tracking of the pathways will also facilitate the Trust in achieving National Cancer Waiting Times Standards, National Cancer Peer Review Measures and other appropriate standards
