**The Role**:
To support the team leader in the nursing management of the complex care caseload. This will include the assessment, planning, implementation, and evaluation of nursing care for children with complex health needs within the community. To support and undertake Children’s Continuing Care Assessment and Review.

The post holder will demonstrate leadership and delegation skills to ensure holistic care is delivered. To take charge in the absence of the Team leader including day to day management of the Senior Nursing Assistants and Nursing Associates

**What you’ll be responsible for**

The role will involve, but not be limited to you performing the following:
To assess, plan, implement and evaluate programmes of care in accordance with the Trust agreed clinical standards.

To promote clinical excellence through principles of trust’s philosophy.

To support the team leader in ensuring the compliance of quality assurance initiatives.

To participate in the identification of the health needs of the child with complex care needs in the community and to work in collaboration with other health care and multi-agency workers to adopt a multi-disciplinary approach to health care and create seamless care.

To develop the role of the community children and young person’s complex care team in accordance with the needs of the client group, the service and the NMC

**You’ll have the following qualification/experience**:

- A minimum of 6 months paid experience in an Acute hospital environment within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- To recognise and identify educational needs of self and team members and wider Children’s community nursing services. To support the team leader in the encouragement of a learning environment.
- To maintain personal and professional development and participate in the annual PDP process
