Job Description:
This is an exciting opportunity for an experienced Diabetes Specialist Nurse to join our team. As a Band 6 nurse, you will be responsible for assessing, planning, prioritizing, implementing, and evaluating nursing interventions to meet patients' needs. You will also provide education and training to patients, carers, and healthcare professionals on diabetes management and care.
Key Requirements:
- A minimum of 2 years practising as a diabetes nurse.
- NMP qualification.
- Competence with all Continuous Glucose Systems.
- Experience with both primary and secondary care in relation to diabetes management.
- Ability to communicate complex information effectively to patients, carers, and healthcare professionals.
Working Arrangements:
You will be required to work across sites, including Wythenshawe, Wythenshawe and Trafford hospitals. The trust offers flexible working arrangements, and you will be expected to work collaboratively with colleagues to achieve excellent patient outcomes.
