We are seeking to recruit additional team members to cover the South Oxfordshire area.

**Hours;** Part time and full time (21-35) hours available working, Monday - Friday, school hours may be considered

**Salary;** £12.67 - £13.18 per hour

**Position**:
**Job Purpose**:
To support and empower people to **‘live life to the full’**, by maintaining their independence, health and wellbeing.

Working as part of our **Community Links Oxfordshire Team**, you will work within local communities across Oxfordshire, with individuals, families and groups listening to what matters to them. Connecting people to and improving access to information, support, activities and opportunities for all those with a health, care or support need. We work closely with local health and social care partners.

You will predominantly be working from home, with some work in community settings, hospital settings or working with people in their own homes.

Every day is varied and interesting. This is a blended role, so you will get to work across at least two different workstreams within the team. We have a strengths-based approach with the people we work with and our team. We will help you to develop your skills and excel.

Our workstreams include:

- Discharge Support at the John Radcliffe, the Horton Hospital and Community hospitals
- Social Prescribing working in GP surgeries
- Networker roles within Adult Social Care Teams and Oxford Health’s Ageing Well
- Community information events and community activities

**MAIN DUTIES**:

- Support individuals and their families by taking a strengths-based approach, providing telephone and face to face information and signposting in both community and hospital settings, and in people’s own homes. Supporting them to create an individual plan as appropriate, to help people achieve their goals.
- Keep information and advice knowledge up to date by attending regular training sessions and team meetings.
- By being responsive and creative, develop, promote and deliver a menu of sustainable activities and opportunities for older people within their community.
- Help improve access to available services, supporting people to know about and link into the support that exists through community-based network events, information fairs, drop-ins and talks.
- Ensure that local resources within communities are mapped and work to further develop the network of grassroots partnerships with local organisations, individuals and groups.
- Work with, further develop and sustain the community link/volunteer network, building and supporting a team of volunteers to help run activities and support individuals to access opportunities in the locality.
- Ensure that systems are in place for receiving and recording regular feedback from people using the service, to ensure the service is of high quality and meeting people’s needs/
- Maintaining accurate and up to date casework records and contributing towards the effective monitoring and review of services.

**Requirements**:
**Qualifications/Education/Training**

**Essential Requirement;**
- 5+ O levels/GCSE/CSE or equivalent; grades A-C including Maths & English;

**Relevant Experience**

**Essential Requirement;**
- Experience of working with older people and an understanding approach to their needs
- Experience of working collaboratively with other statutory and voluntary agencies
- Experience of providing relevant information and support to vulnerable people

**Desirable Requirement;**
- Experience of working in the Charity environment and / or working with Volunteers and/ or in the community;
- Experience of working within a health/social care setting

**Relevant knowledge/Skills;**

**Essential Requirement;**
- Excellent oral and written communication skills, able to relate to people at all levels and in differing circumstances

**Desirable Requirement;**
- Experience of using a strengths based approach
