Zion care services Ltd is a care company providing care to the local communities, most vulnerable members across the midlands. W**e are now looking for an experienced Care Coordinator to join a busy and thriving domiciliary care company in Wolverhampton.**

**The care coordinator must be able to drive and have his/her own car.**

This is a varied role and will include working closely with senior member of staff and management team.

You’ll need to be tech savvy as you’ll be using a number of software packages for the coordination and recording of the care services. **Prior experience as a Care coordinator would be advantageous.**

The main responsibility will include:

- The coordination of the rotas for the care staff. Ensuring that full cover is provided at all times and that the rota is effectively communicated to the care staff.
- Under take out of hours telephone call.
- Cover care calls and input data as required.
- Reallocating care visits when staff are absent, ensuring that any changes are communicate to the client, or deal with any queries or concerns from service users or staff
- Also liaise regularly with health care professionals such as GP’s, District Nurses and social workers.
- You’ll have excellent IT skills and the ability to use various software packages
- You’ll have a can-do attitude and have the skills to lead and motivate people.

**Salary**: From £24,000.00 per year
