Excellent problem solver wanted We are looking for a mental health social care support worker who can transform the lives of people with mental health conditions who are ready to be discharged from hospital.

A friendly and dynamic integrated mental health team in the beautiful City of Oxford is looking for a mental health support worker to work in partnership with people who use mental health services. You will work alongside the inpatient mental health social workers and the wider adult mental health team to provide targeted support to people receiving care and treatment for their mental health difficulties.

You will support the team by addressing any non-clinical barriers and supporting discharge plans for social care needs of people receiving care and treatment within the Acute Wards at the Warneford Hospital and Littlemore Mental Health Centre.

Sometimes the work is stressful, and you may be working with situations that have high levels of complexity, risk, or vulnerability. You will be well supported by your colleagues and you will have monthly supervision. You will be a very important part of the mental health social work group and you will be invited to attend the monthly social work forum is where our energetic collective meet to share ideas and talk about the messy stuff.

1. To provide support and give time to an allocated group of service users in order to promote their recovery and maintain them in a community environment helping them to build their own support network.
2. To provide regular and practical support to service users in developing and managing independence and maintaining dignity and self-respect.
3. To support service users to identify their needs and to assist the mental health social worker to assess, plan, implement and evaluate individual care and support plans.
4. To have the individual service user’s needs at the fore at all times and use agreed values and skills to underpin their day to day work.
5. To work as part of a team under direction of the mental health social worker to focus on the direct needs of service users, working across boundaries of care, organisation and role, co-ordinated through the Care Programme Approach, Care Act 2014 and s.117 aftercare Mental Health Act.
6. To build rapport with service users this will include working with them and developing a working relationship.
7. To help ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individuals. To support service users to engage effectively with the agreed CPA and care and support plans and access appropriate services.
8. To positively promote independent living of service users within the community by maintaining extensive knowledge and links with community resources and actively supporting service users to use them e.g. accessing clubs, centres, libraries and daytime services. Where appropriate liaise with other agencies e.g. Benefits Agency, Housing Department, gas and electricity services.
9. To provide support with daily living including self-care and home management e.g. budgeting, cooking, cleaning and shopping in line with the person agreed eligible needs.
10. To help service users to access information about resources including, for example, health services (physical health) benefits, welfare rights and health promotion topics.
11. To contribute to the delivery of CPA planning by providing regular and practical support to service users and their carers.
12. To encourage service users to act in response to any changes including relapse.
13. Or, if needed, in the interests of the service user’s health or safety or the protection of others, for the mental health social care support worker to initiate appropriate action, within or
