An opportunity has arisen to join our new Frailty Virtual Ward team. This is a system level collaboration between WUTH, WCHC, CWP, Wirral ICB and Healthy Wirral.

The post holder will be involved in the initial assessment including taking medical histories; assessing and examining patients; initiating and interpreting investigative laboratory, imaging and diagnostic tests; recommending appropriate patient management and presenting patient data and plans to the supervising GP.

The PA will provide care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The PA will provide a holistic and clinical service, with support from GPs as required, implementing agreed management plans and following approved protocols as appropriate.

The PA role involves working alongside and supporting the existing Community Integrated Response Team (CIRT) and Community Geriatrician in managing frail older patients who are referred to the service. We are also looking to expand the current team with clinical fellows (SHOs), ANP and GPs. The CIRT team will be expanding their existing team with additional nurses and therapists. It already has a complement of nurses and therapists who are undergoing continued professional development to reach Frailty advanced practitioner competency. The service links very closely with our Older Person’s Rapid Assessment Clinic to provide timely diagnostics where appropriate.

Wirral University Teaching Hospital is a large district general hospital serving a population of 350,000 on the Wirral peninsula. The area has a high proportion of older patients compared to the national average and has a significant care home population resulting in a high number of patients with frailty. As part of the NHS Long Term Plan (LTP) the trust is continuing with development and expansion of its Community Geriatric services to support its innovative, multi-disciplinary community care services across Wirral. The NHS LTP makes clear the vital role community services will have in the future delivery of ‘out-of-hospital’ care, including an enhanced urgent community response and support for recovery, long-term condition management and preventative care, closer links between Primary Care Networks and Social Care, and enhanced care home support.

Wirral Trust’s Single Point of Access provides a front door for referrers to access the District Nursing teams and Community Therapists. The Community Integrated Rapid Response Team (CIRT) is also accessible through this contact. CIRT work closely with our existing Community Geriatricians to provide rapid Comprehensive Geriatric Assessment of patients, who have had an acute decline, in their community dwelling as an alternative to hospital admission.

**Key areas of the role would include the following**:

- Triaging referrals
- Prescribing of medications
- Providing Comprehensive Geriatric Assessments
- Liaising with Community Geriatricians
- Carrying out video and telephone consultations with patients and carers
- Some face-to-face visits for more complex cases
- Anticipatory care planning
- Escalation plan discussions
- Advice and leadership for team in absence of Consultant Community Geriatrician
- Documentation and summarising
- Supporting CPD and professional development of the team

You will be given training to manage the hospital’s electronic patient record, called Cerner Millennium, to enable electronic recording of patient assessments and the requesting and endorsing of investigations.
