Health & Wellbeing Coach (Mental Health)

Have a thorough general knowledge of the voluntary sector and charities that are available in Penwith specifically, that aid bio/psycho/social wellbeing. Have excellent communication skills and be able to use these to effectively care for our patients in collaboration with the multi-disciplinary team and their own GP. An understanding of the trauma informed approach together with an understanding of health and social inequalities experienced by the population of Penwith. Experience of working within a team environment but also working autonomously.
Some outreach work and lone working in patients homes may be necessary for those who would find attending the centre too challenging initially. An ability to feel comfortable in times of change and uncertainty in a working environment. Basic IT skills (training will be provided on the GP patient record system and that of secondary care). Undertake mandatory training online or face to face and be willing to undertake more specialist training.
This will include learning to facilitate groups within the centre such as emotional coping skills (ECS). To enrol on and undertake coaching qualifications. An understanding and adherence to patient confidentiality/data protection without compromise, according to associated policies and procedures across the PCN. The desire, skill, and ability to develop the new centres in line with Cornwalls Mental Health Transformation Plan.
You will be instrumental in triaging patients using a coaching conversations method. You will then ensure they are either signposted to services we/you offer, given a clinical appointment with a senior mental health practitioner or other partner services in West Penwith. You will be comfortable with assessing risk and ensuring you communicate any concerns to the wider MDT. Desirable skills and abilities Be able to bring perhaps their own, additional qualities to this role that might aid wellbeing for our patients.
This may include enhanced communication skills, complimentary therapeutic approaches, counselling qualifications, work with charitable organisations or sports/health promotion skills (the list is endless). A basic understanding of medication, physiological comorbidities, and the effects of these on a persons presentation would be desirable. Knowledge and understanding training (KUF framework) training. Fortnightly MDT together with secondary care and wider MDT colleagues. Centre based working across Penwith in Hayle, St Ives and Penzance. Possibility of occasional home and community working too.
Flexible and adaptable approach to work/life balance supported by your line manager. Two safety huddle meetings a day. Part time or full-time hours and extended long days may be considered. You get to work with a fun team who all support each other every day
