Under the guidance and supervision of the DCH Clinical Team, the post holder will work as a member of the Hub team to undertake a range of duties which include: taking calls from Care Home staff, registering residents from care homes on to SystmOne, actioning tasks allocated by clinical team, supporting the training of care home staff and maintaining own competence in documentation, education support and direct care activities to support care home staff in care homes. The post holder will work to the Level 3 standards and complete the Trust competency booklet, having completed the care certificate.

The Digital Care Hub provides video and telephone support to over 22,000 patients living in their own homes and in nursing and residential care.

The Hub is a 24/7 service, with the aim of supporting patients in their own place of residence where it is clinically safe to do so, reducing the number of unnecessary attendances at A&E and unplanned admissions.

The Hub provides a range of services alongside supporting care homes with the Immedicare Service, end of life patients through Gold Line, and patients with long term conditions through the MyCare24 services.

The service is rapidly expanding and as such we need additional staff to offer greater support to the new innovations in our work.

Applicants will need to have an innovative approach to work and be receptive to constant change.
