We are a innovative Primary Care Networks that serve the population of Leigh-on-Sea, Essex. We have recently developed and delivered an integrative model of working to help support our frail patients and are now looking to transfer the basis of this model to our patients experiencing mental health
The role will require the post holder to contribute in designing and delivering our vision for a truly collaborative model of working, providing a patient focused service and building strong professional relationships. You’ll support delivering effective and expert Mental Health advice, ensuing compliance and good practice in line with our governance and procedures, and the wider government agenda.
The post-holder will work closely with our community and acute sector partners to deliver pro-active and re-active care. The post-holder is expected to be an experienced mental health practitioner and with experience in transformation and project experience within an NHS environment.
The post-holder will help the PCN deliver new initiatives to improve access to care and enable PCN staff to work more effectively to support the sustainability of general practice services
You will be a qualified Mental Health Nurse with NMC registration.

**About Us**:
SS9 Primary Care Network is a collaboration of 5 GP Surgeries in Leigh-on-Sea and covers around 56,000 patients.
Southend is at the forefront of the move towards service integration and our practice staff work very closely with a number of healthcare professionals including; district nurses, midwives, dementia services and drug and alcohol services.
Leigh-on-Sea is set in a picturesque setting, with an historic Old Town and within walking distance to the beach. The town has excellent transport links with two lines to London (just 40-50 minutes by train) and a thriving international airport.
