We are looking for an experienced and enthusiastic individual to work within the LUHFT Cancer Services team leading the delivery of the Personalised Care agenda for patients with cancer across Liverpool in line with the NHS long term plan.

The post holder will have specific responsibility for embedding, sustaining and the ongoing evaluation of the delivery of the Personalised Care agenda working collaboratively with all clinical teams providing cancer care across LUHFT.

The LUHFT Personalised Care Lead is an exciting new role within the Trust designed to progress, support and develop the Personalised Cancer Care offer for all patients with cancer and their carers' to ensure delivery of a high quality service.

The post holder will be expected to work across all the Royal Liverpool, Broadgreen Hospital and Aintree sites.

The post holder will have a proven track record in project management, leadership skills as well as excellent communication and networking stakeholder. The ability to formulate strong internal and external working relationships is a must for this role.

The post holder will be responsible for leading, developing and delivering relevant workstreams responsive to the growing needs of the cancer patient population across Liverpool. This will include collaboration with acute care providers, primary care and 3rd sector organisations across the Liverpool City region footprint.
- Coordinate the delivery of the cancer community support service across LUHFT under the supervision of the Lead Nurse
- Support clinical teams with education and training relevant to personalised care interventions
- Provide expert signposting for patients and loved ones living with cancer along with those delivering cancer care
- To lead and facilitate the delivery of personalised care interventions for complex cancer discharges working closely with the LUHFT discharge planning teams
- To propose changes to processes, procedures and patient pathways relation to personalised care interventions and lead such change where appropriate
- Lead the engagement around personalised cancer care with multi-disciplinary teams, care groups and specialities, working with operational and clinical team members
- To be involved in the development, creation and review of any relevant standard operating procedures and policies to support the delivery of personalised care
- To ensure training and induction packages are in place for all staff groups in relation to personalised cancer care.
- To deputise as appropriate for the Lead Nurse for Cancer Community Support as required
- To attend and contribute as a key member of the cancer services team to operational and governance meetings having responsibility for any actions regarding personalised cancer care

- Provide project management expertise and advice within in the Directorate/organisation
- Have direct responsibility for a number of projects, for the full lifecycle from initiation to project closure, taking a lead role on high priority/critical projects.
- Oversee the successful delivery of a number of projects managed by members of the wider project team.
- Ensure that all projects are managed in accordance with PRINCE2 Project Management standards.
- Ensure that each project is supported by a robust governance structure.
- Monitor the progress of the project against the original business case and project initiation document, ensuring where necessary the project is able to adapt to changing requirements and that plans are adjusted accordingly so that the deliverables are on time, to specified quality and within budget.
- To analyse and evaluate many highly complex and diverse conflicting issues associated with the completion of a project, which requires the analysis,

interpretation and comparison of a range of options and decide how resources are to be used. Including within this is the management of key interdependencies with other projects.

Be responsible for developing and tracking the progress of project plans, including resource planning, using approved software.
