**The Role**:
This is an exciting opportunity to support the current delivery of improvement programmes for patients living with frailty. This role will specifically support a Frailty Test of Change in the ACE team as a CSW, within wards 2 and 3.

**What we'll need you to bring**:

- Being able to assist with process of admission, transfer and discharge of patients.
- The ability to perform phlebotomy and cannulation
- Assist in the process screening/identification/ and treatment of Delirium
- Assist in obtaining collateral history in the context of Comprehensive geriatric assessment.
- Providing escort duties as required.
- Undertake recording of patient vital signs and report results to registered staff.
- Assist in supporting and promoting patient mobility in conjunction and in support of other members of the multidisciplinary team to continue rehabilitation.
- Report observed changes in the patients physical and psychological needs and participate in maintaining accurate and up to date records, to ensure effective communication.
- Assist with operational flow: attend twice daily board rounds with MDT, work with team members to facilitate safe and effective discharge/getting right patient to right place at the right time

**It would be great if you also have**:

- Experience and/or interest Frailty and COTE patient pathways and discharge planning
- Experience within Care of the Elderly
- Effective listening, communication and interpersonal skills.
- Ability to work with people and as part of a multidisciplinary project team.
- Excellent leadership skills with the ability to work using own initiative.
- Ability to support new & innovative ways of working

**Contract type**:
Fixed Term or Secondment for 12 Months

Full time

37.5 hours

This role will be based in Wards 2 and 3 within University Hospital Hairmyres.
