Community Staff Nurse

The post holder will be part of the 0-19 Children’s Health and Wellbeing Service providing public health advice, information, guidance, and referral and providing early intervention. In doing so, they will contribute to the assessment, planning, delivery and evaluation of programmes of health, ensuring that they practice within their sphere of competence and
knowledge.
They will work to professional codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.
This is a dynamic post that requires the Staff Nurse to work closely with uni/multi -disciplinary teams in addressing a range of care and support issues across a spectrum of needs and preventing unnecessary hospital admissions. The nature of the service
will be determined by local service specifications.
Training will be provided to achieve the necessary competencies
- Assessment of child growth
- weigh and measure babies and young people
- Competent in infant feeding and nutrition and starting solid foods
- Competent in assessment of child development
- Competent in public health advice, delivery home safety, vaccinations, dental care, safe sleep, behaviour
- Assessment of maternal mental health and referral onwards
- Knowledge of safeguarding and child protection
- Enuresis and child continence/ constipation

The post holder will promote the health of children aged 0-19 within the Staffordshire area through the public health agenda, thus enabling each child to reach his/her development, and educational potential.

Formulates, develop, implement and evaluate programmes of care and support. The
**care and support plans may include the following**:
- Promoting independence/self-management and maintaining safety
- Improvement in health and wellbeing including mental health
- Interventions in disease/condition management
- Prevention and reducing of health inequalities
- Admission avoidance
- Case management/ treatment/ care plans
- To weigh and measure children, young people and infants and record accurately
- To support the public health practitioners with the completion of caseload profiles
identifying health needs
- Health promotion, advice and guidance
- Signposting to other services
- Contribute to the health profiling information and local data in line with key performance indicators
To use professional knowledge and judgement to take into account the range of
care/support packages and treatment options available. To consider the personal
circumstances and preferences of children, young people and families in order to
implement and adjust programmes of care
