To offer support for people experiencing mental health difficulties.

You will work in a recovery focused way, whilst assessing risk, working in collaboration with the service user to determine the best way to meet their recovery goals.

To work within a team setting to offer care coordination for service users that are experiencing mental health difficulties. You will manage a caseload, offering assessment, risk assessment, interventions and care planning to support their recovery. You will be part of a wider multi-disciplinary team including nurses, social workers, OT’s, medics and psychologists working collaboratively to provide specialist focused mental health interventions.

You will work alongside the wider community provision in Leeds, and will refer for relevant support to aid ongoing recovery.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. With access to appropriate clinical supervision the post holder will have or will develop and will be able to demonstrate in practice a range of appropriate skills to include clinical and risk assessment and treatment interventions as determined by Integrated Care Pathways.

- To demonstrate appropriate judgement in unpredictable situations of risk and manage these in a safe, professional and holistic manner using FACE risk assessment and management plans and understands the need to seek appropriate advice and suppor
- Will be able to demonstrate a growing level of understanding of the national and local mental health and social care agenda and be able contribute to service development initiatives
- Will be able to demonstrate a working knowledge of clinical, teaching and leadership skills and can increasingly demonstrate evidence to support same.
- Will be competent in all aspects of clinical communication including the maintenance of accurate records and will show a commitment to further develop these skills.
- Will be able to demonstrate appropriate prioritisation of own workload.
- Will have evidence of continuous personal and professional development and be actively involved in case load management supervision, clinical supervision and clinical audit within the team
- Demonstrates awareness of role with regard to the quality of care delivery and standard setting process and is able to promote this to peers
- Up to date knowledge of service user developments in relation to therapeutic interventions
- Knowledge of therapeutic approaches to care
- Demonstrates flexibility in approach to clinical work and resource management
- Demonstrates a good understanding of accountability and responsibility
- Actively seeks out and participates in clinical supervision, managerial supervision, PDP and KSF and provide these to junior staff if appropriately skilled and experienced
- Shows a positive attitude and approach towards own personal development
