The Acute Frailty Team currently provide an in reach service into the Emergency Departments (ED) at both our St Mary’s and Charing Cross sites, assessing frail patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance.

- To use the CGA knowledge gained, initiating and advocating plans which not only enables an early discharge but also meets the needs of the patient and reduces the likelihood of an unnecessary readmission
- The role requires understanding of the particular requirements of elderly patients within an acute setting and the knowledge and skills to assess patients with commonly recognised admission criteria E.g. Falls, Urinary Tract and Lower Respiratory tract Infections, Delirium and Dementia.
- The role demands the commitment to provide clinical support to patients, their families/carers and health professionals involved in the patient journey. This support will when appropriate involve discussions on Universal Care Planning
- The post holder will advance the role by integrating the four key pillars of advanced clinical practice, service management, education and research whilst collaborating closely with other members of the multi-disciplinary team to develop and monitor standards of care.
