Band 4 Assistant Practitioner - Band 3 to 4 development opportunity

The Oxford University Hospitals NHS Trust is one of the largest NHS teaching trusts in the country. The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington, Oxford and the Horton Hospital in Banbury. The Adult Diabetes Service covers both in
- and out-patients and provides clinical, support and education services to people living with Diabetes and to other clinical staff across the four hospitals.

The Diabetes MDT has an exciting opportunity for an Assistant Practitioner to join their team. The role will consist of working alongside the members of the Specialist Diabetes MDT to implement and support agreed treatment plans both in the outpatient setting and during their stay in hospital.

Main duties will consist of collecting 8 care processes, educating people with diabetes in self blood glucose monitoring, insulin therapy, use of insulin delivery devices which may include insulin pumps and delivering education packages in relation to their diabetes management; the post holder will also be involved in assessing the educational needs of Heath Care Professionals and delivering educational programmes to these needs. This role will also support outreach work for visits for people at home when required.

The Diabetes Assistant Practitioner will also assist the diabetes specialist MDT in supporting people living with Diabetes in other departments including podiatry, haematology, oncology, renal/dialysis, ambulatory care and other specialities
