**We currently have an exciting opportunity to join our substance misuse team in Devon as a Community Transitions Worker.**

As a Community Transitions Worker, you will be part of an established and passionate team, working with individuals to support them as they transition between prisons and the community, to ensure continuity of care and support with their ongoing treatment.

You will work closely with street homeless teams, the Police, and Community Safety Teams, to ensure that our service users are given every chance to succeed when re-entering the community and engaging with the community treatment services.

**What does a day in the life of a Community Transitions Worker involve?**

This role is incredibly varied and busy, but it is also incredibly rewarding. You will hold a caseload of service users that you will work with for a period 12 weeks, leading up to and after their release from prisons. The duties include:

- Utilising a trauma sensitive, warm and non-judgemental approach, to develop strong collaborative therapeutic alliance.
- Undertaking risk assessments and risk management planning in consultation with colleagues and partner agencies, as appropriate.
- Delivering interventions in a range of settings including harm reduction, 1:1 support, group work, guided self-help and intensive structured programmes.
- Working from a variety of locations, including hubs, satellites and partnership sites, including Criminal Justice agencies and outreach.

**Position Details**:
This role will be working with individuals who access our services and are due for release from prison in all Devon Prisons, HMP Exeter, HMP Channings Wood and HMP Dartmoor.

The role can be based from any of the above prisons or our community hub locations; there will be an element of travel between hubs, prisons and partnership sites. Mileage is fully paid for travel between locations.

This role is available on a full time basis, working 35 hours per week, Monday to Friday.

**What skills are we looking for when you join our team?**

Most importantly, you will be someone with a genuine interest in the understanding of substance misuse problems and a positive, recovery focused attitude.

We are looking for someone ideally with experience in a similar role or environment who is looking for an opportunity in a chance to make a difference and a chance to use your transferable skills.

**Additionally, you will have**:

- Experience handling a caseload, undertaking assessments and working with individuals to develop, implement and review action plans.
- Knowledge and understanding of the criminal justice system, including the National Probation Service, Community Rehabilitation Companies, Courts, Police and Prisons.
- Excellent communication and rapport building skills, able to communicate with a range of individuals, including children, families and carers.
- Awareness of current national/local guidelines and protocols on substance misuse, treatment and recovery, including the importance of the role of community in supporting recovery.
