An exciting opportunity has arisen to join the Adult Integrated Care Dietetic Team at Guy's and St Thomas' NHS Foundation Trust. We are seeking 1.0WTE Band 4 Advanced Dietetic Assistant, to be eligible for this position you must have excellent communication skills and a strong interest in nutrition and of providing good nutritional care to patients who are at risk of malnutrition. The post requires excellent communication skills as you will be working with a wide range of patients, carers and healthcare professionals to provide advice, training and individualised care plans for patients referred to the service. You will also provide training to multidisciplinary teams and carers on malnutrition screening and nutrition support.

You will be working with a wide range of people including patients, carers, nursing teams, therapy teams and GPs. You will provide direct care to patients and education surrounding identifying and treating malnutrition to local teams including the voluntary sector. Good listening skills, attention to detail and an ability to collect and record information accurately and concisely are essential. You must also be able to communicate nutrition care plans to patients and carers clearly and you will be responsible for reviewing the implementation and progress of care plans.

You will be required to assess patients in care homes and in their own homes across the boroughs of Lambeth and Southwark and will be working with supervision from the registered dietitians in the team.

We are a large Academic Health Sciences Centre with King’s College London and King’s College Hospital NHS Foundation Trust. This is a unique opportunity to work alongside experienced clinicians and leading researchers in the Trust and at King’s College London.
You will be joining a department of well over 100 staff working across Guy's, St Thomas’, Evelina Childrens Hospital and Lambeth and Southwark community services. The department has a strong commitment to staff development.

To act with professionalism at all times

To contribute to enabling a safe and therapeutic environment for patients and their carers and relatives.

To be responsible for designated aspects of care for patients residing in community settings under the direction of a registered Dietitian.

Under indirect supervision, perform assessment of patients’ nutrition needs in acute and community settings and plan and deliver high standards of care.

Under indirect supervision, design, deliver and audit outcomes of education and training to members of the MDT, patients and/or carers in acute, community and care home settings.

To prioritise care provision and caseload as appropriate.

To demonstrate excellent communication skills when dealing with patients, carers, relatives and members of the multi-disciplinary team (MDT) demonstrating sensitivity, compassion and ability to manage conflict.

To keep clinical and non-clinical staff informed of patient status and seek advice and support where required.

To recognise changes in a patient’s condition and report promptly to relevant dietitian/member of the MDT.

To immediately escalate any concerns to a registered dietitian/AHP/healthcare professional.

To immediately escalate any clinical incidents to a registered dietitian/AHP/healthcare professional.

To confidently speak up when witness or feel that care is not being delivered in a safe, caring and compassionate way.

To contribute to effective and timely discharge planning.

To perform, accurately record and interpret patient observations. This may include fluid balance, nutritional intake, and any other nutrition related factors important for assessment and nutrition care plans.

To perform, understand and report patient risk assessments relevant to dietetics and patient population.

To perform additional tasks specific to nutritional care as directed by the registered dietitian/healthcare professional and following completion of appropriate training and relevant competencies.

Act in accordance with the Code of Conduct for Healthcare Support Workers.

To liaise with other relevant health and social care professionals e.g. AHPs, nursing, and medical staff re plan for patient and to keep the registered dietitian/healthcare professional informed.

Provide concise handovers to other members of the MDT using SBAR (Situation, Background, Assessment and Recommendation).

To be aware of the specific physical, psychological, social, cultural and spiritual needs of patients and implement appropriate strategies to care for these patients in conjunction with the registered dietitian.

Maintain monitoring of equipment contracts and teaching others about specific equipment related to role.
