Understanding the health and care needs of the people of Sheffield is key to the delivery of high quality care and reducing inequalities. This post will work with the Clinical Effectiveness, Quality Improvement and Research and Innovation teams, amongst others, to deliver our Health & Social Care Strategy - particularly our strategic priority of working in partnership to have bigger impact and our strategic aim of ensuring our services are inclusive.

With a background in public health and/or health inequalities, the postholder will work with other leaders to develop, implement, and deliver of our Health and Social Care Strategy. Taking action to reduce health inequalities is a strategic priority for Sheffield Health and Social Care and this is an important and exciting role. The postholder will cultivate trusted and impactful relationships across the system and in our communities, understanding, enabling and involving people who may be affected by inequality or exclusion. They will lead on the use of data, analytics and intelligence to understand the different needs in communities and the health inequalities present, to ensure that this becomes an embedded part of business as usual in our service development, service delivery and business planning work.

- Develop and implement a population health plan, working with colleagues in SHSC and throughout the system, to ensure that it brings demonstrable outcomes, in line with Trust values.
- Review, establish and lead the resource and governance arrangements needed to drive the population health plan and health inequalities agenda in SHSC.
- Work with colleagues in Clinical Services, Clinical Effectiveness, Quality Improvement and Research to use data to identify opportunities to develop bespoke interventions for people affected by health inequalities.
- Work with our partners in the city to achieve an impact on health inequalities. Representing SHSC as an expert in population health and ensuring that we have appropriate and representative involvement in community groups.
- Working with the Clinical Outcomes Lead, create appropriate systematised measurement of impact and support the collection of data and information for service development in line with requirements.
- Work with the services and the Service User Engagement Team to ensure that we appropriately enhance our community engagement in ways that are effective, to mitigate the risks associated with relevant protected characteristics and social and economic conditions, ensuring person centred care and effective engagement with those communities experiencing the greatest health inequalities.
- Develop links with the Occupational Development Team to ensure that staff across Health and Care Services are equipped with the requisite skills and competencies to deliver on the population health and inequalities agenda.
- Work with the Leadership Teams of Services across SHSC to ensure that our services are developed and delivered inclusively and in ways that best serve those patients and communities at greatest need and that future transformation is done in a way that improves the health of those who need it the most.
- Work with the services to ensure they offer equitable access, great care and optimal outcomes to those experiencing the greatest health inequalities including developing digitally enabled care pathways in ways which increase inclusion, ensuring face to face options continued to be offered.
