Humphrey House supports people whose mental health problems 18-65 in the community are affecting their ability to carry out their normal everyday activities and hobbies. Humphrey House supports people managing the symptoms of their mental health problems and coping with the changes in life that have been caused by their mental health problems. Humphrey House gives people the opportunity to learn more about mental wellbeing and explore activities and techniques which allow them to maintain their independence and their contacts in the community. Humphrey House offers a range of activities and therapeutic groups which help patients to regain their confidence and manage their symptoms including wellbeing groups and anxiety management. Our aim is for patients to work towards managing their symptoms and regaining their skills and confidence in everyday activities.

The post holder will work as part of Humphrey House CMHT, to undertake allocated duties within the team.
Working alongside mental health practitioners, Social Workers, OTs, families and carers in the community and also supporting patients who are on the discharge pathway from the CMHT developing interests and opportunities in the community and ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.

To provide support to the CMHT service users, promote their recovery and help to maintain them in their community environment.
- To assist qualified practitioners in assessment, planning, implementing and evaluating individual care plans.
- To ensure that service users engage effectively with the agreed Care Plan and access appropriate services.
- Positively promote independent living of service users within the community
- Provide support and interventions to service users both at the day hospital, in their own homes and the wider community based upon their assessed needs.
- Assist service users to make full use of community resources.
- Support service users to become involved in social activities in line with their assessed needs and planned care.
