**Job Title: Outreach Nurture Mentor - SEMH Support**

**Location**: ST HELENS

**About Us**:
We are a dedicated team committed to providing exceptional support to children with Social, Emotional, and Mental Health (SEMH) needs. Our outreach program extends beyond the traditional school setting, offering personalised support to children in various environments. As we continue to expand our outreach services, we are seeking a compassionate and experienced Nurture Mentor to support a child with SEMH needs outside of the school setting.

**Key Responsibilities**:

- Establish a trusting and supportive relationship with the child, building rapport and understanding their individual needs.
- Design and implement personalized support plans tailored to the child's SEMH needs, in collaboration with the child, their family, and other professionals.
- Provide emotional support and guidance to help the child develop coping strategies and resilience.
- Encourage positive behavior and social skills development through structured activities and interventions.
- Work closely with the child's family, offering advice, guidance, and support to promote consistency and positive outcomes.
- Collaborate with other professionals involved in the child's care, such as therapists, counselors, and educational professionals, to ensure a holistic approach to support.
- Monitor the child's progress and adjust support strategies as needed, providing regular feedback and updates to relevant parties.

**Requirements**:

- Previous experience working with children with SEMH needs, either in a school or community setting.
- Strong understanding of SEMH challenges and evidence-based strategies for supporting children's emotional and behavioral well-being.
- Excellent communication and interpersonal skills, with the ability to build positive relationships with children, families, and professionals.
- Empathy, patience, and a nurturing approach to supporting children's social and emotional development.
- Flexibility and adaptability to work in different environments and accommodate the child's needs.
- Relevant qualifications or training in areas such as counseling, psychology, or social work are desirable but not essential.
