Camhs Mental Health Care Navigator

An opportunity has arisen within the Milton Keynes CAMHS team based at Eaglestone Health on the hospital campus for an experienced administrator to take on the role of Care Navigator.
Currently the service has 2 Care Navigators in post, with increasing workloads a vacancy has arisen to take on another member of the team. The key role of the Care Navigator is to support the clinical team with tasks centred around the care of children and young people who use the service. This can range from onward referrals, minute taking at professionals meetings or general administration tasks. The role offers a high degree of autonomy and variety where no 2 days are the same.
The post holder will work closely supporting our nurses, therapists and psychologists with non clinical aspects of a young person journey through the service. The post will also involve supporting the Patient Pathway Action Group meeting to take notes and actions from the meeting and ensure those actions are completed efficiently. The role would be best suited to an experienced administrator who is looking for something different to a generic administration role and someone who is proactive in implementing systems and ideas. Previous experience working within a healthcare setting would be advantageous.

Role provides prompt, efficient coordination and administration support to a multi-disciplinary team (MDT) within the Core CAMHS team
Works with relevant MDT professionals to support implementation of care provision and tracks progress of care
To ensure a prompt response to referrals and co-ordinate interventions by team members or ensure referrals are made promptly to other services to ensure appropriate and timely action
To maintain the information systems used by the team in order to collate and co-ordinate performance data and produce reports as required for the team and relevant managers
