The East Suffolk and North Essex Foundation Trust recognises to meet the needs of the population it serves, a multidisciplinary workforce that delivers and promotes advance practice, skills and knowledge is essential. Therefore the Older Persons services are looking to expand its medical cover across the community hospitals to include the Advanced Clinical Practitioner role.

You will be integral in supporting the Multidisciplinary Team in the provision and delivery of the day to day acute and complex care within community hospital setting. You will be a pioneering practitioner who is clinically sound with up-to date knowledge of older person’s medicine and care, providing the highest standard of individualised care to patients.

The Advanced Clinical Practitioner (ACP) will use a high degree of autonomy and complex decision making alongside demonstrating the four pillars of practice, Clinical Practice, Leadership and Management, Education and Research in providing healthcare that enables innovative solutions to enhance experience and improve outcomes within their speciality area.

The ACP is accountable for independent clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice. This encompasses a comprehensive assessment of patients with diverse minor injury, minor illness and urgent presentations and develop working diagnosis and treatment plans whilst working without direct supervision.

In addition to this the ACP is also responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.
