Work with a high degree of autonomy and complex decision making Act as a source of clinical expertise, knowledge and skills in advanced clinical practice that is accessible, flexible and provides a high quality service to enhance patient care. Take a lead role in the advanced clinical assessment of patients who have highly complex and or chronic disease presentations, including screening patients for disease risk factors and early signs of illness, making a differential diagnosis and prescribing treatments as an independent non-medical prescriber. Develop and maintain effective communication systems to ensure staff receive and contribute information regarding Trust and professional issues and developments. To work with internal and external agencies To attend and chair meetings required by the service To work according to Trust values, policies and procedures Participate in developing clinical policies, guidelines, service plans and pathways for self and others based on evidence based practice.
