Hertfordshire and West Essex Integrated Care Board based in Welwyn Garden City is currently seeking Head of Continuing Health Care to join their team on a full time basis for a minimum for 12 weeks. The Head of Continuing Health Care will support the Assistant Director in delivering the DOH policies related to Continuing Healthcare and Funded Nursing Care, so that the policies are implemented to a high standard which enable consistent decisions to be made on an individual’s eligibility for NHS funded healthcare and then subsequently safe and appropriate clinical care is commissioned monitored and reviewed.

**What you will be responsible for**
- Formulate long term strategic plans, involving uncertainty, which may impact on the whole of the organisation.
- **Support the development of close working relationships with Commissioning Leads, Adult Care Services and private providers across the geographical areas of the CCG**:
- **Ensure effective collaborative inter-departmental working and a culture of success through support; to realise workforce potential in both individual and collective terms; and to operate effective internal communication systems**:
- **Work with senior health and local authority colleagues, including acute trusts, social care and non NHS organisations, providers of specialist health care services in the independent sector. Ensuring complex and challenging clinical and non-clinical problems are evaluated and negotiated solutions implemented.**
The role will involve, but not limited to you performing the above.
**You will have the following qualification/experience**:
- A minimum of 6 months paid experience in a similar role for a UK hospital environment within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.
**Depending on the Trust, you may require variable additional certification and skills.**
