**Job Details**:
Shifts: 07:00-14:30 and/or 16:00-22:30

**Responsibilities**:

- Personal care assistance
- Medication prompting and administration.
- Building meaningful relationships with clients.
- Meal and drink preparation.
- General household tasks, including laundry, shopping, and housework.
- Support in accessing the community.
- Assisting clients in achieving their goals and aspirations

**To succeed as a Community Care Practitioner, you will**:

- Be respectful, empathetic, creative, and caring.
- Possess excellent communication skills.
- Demonstrate reasonable written and verbal communication abilities.
- Be willing to undergo a Disclosure and Barring Service (DBS) check (paid for by HCC).
- Hold a Full UK Driving License and have access to your own vehicle.
- Be available to work ALTERNATE weekends.
