Diabetes Health Care Support Worker

An exciting opportunity has arisen to work as part of an Integrated Diabetes team at the Grange University Hospital. This role will be to support the Diabetes the inpatient care and outpatient clinics, which involves providing foot assessment and patient education under the supervision of the Diabetes Specialist Nurses. We are looking for an individual who is passionate about patient care and driven to working in collaboration with patient, carers and other services to provide an individual, holistic and patient centred approach. The post holder will work as part of the Inpatient diabetes care and education team (DICE)at GUH and to support outpatient clinics at RGH, under the supervision of the Diabetes Specialist Nurses and other health care professionals by participating in the delivery of assigned tasks involving direct and indirect care for the benefit of the person with diabetes.
The post holder will be expected to be self-motivated and use their initiative where necessary. Some of the main duties are; to assess foot assessment and refer to podiatry, to provide blood glucose monitoring education for patients and their family, teaching patients regarding flash glucose monitoring, processing diabetes outpatient clinics and phlebotomy.
The post holder will also have reception and clerical duties which will support the smooth and efficient running of Diabetes service within ABUHB.
