We are looking for a full-time Learning Disability Care Coordinator to join our existing team supporting patients across the Kingstanding, Erdington & Nechells Primary Care Network. This role will involve spending time at practices so travel across designated area of the PCN will be required (the PCN covers the areas B7, B10, B13, B23, B44)

Main duties of the job

As care coordinator you will be coordinating a process to support the needs of patients with Learning Disability or Mental Health Diagnosis.

You will need to build relationships with each practice within the PCN and work closely with other Care Coordinators within the PCN to ensure the needs of patients are met.

About us

Our Health Partnership was set up by local GPs who are passionate about providing high quality primary care and using their time and skills effectively to benefit patients.

We are currently a GP partnership of 30 practices with 39 sites, serving around 280,000 patients in Birmingham, Wolverhampton, and Shropshire.

The partnership offers a shared administrative and management structure, cutting down the time doctors have to spend on admin. It opens up economies of scale to get best value from budgets and can access new funding streams that are only available to large GP organisations. It has the resources to develop innovative services and effective partnerships with local hospitals and care services.

Job description

Job responsibilities

You will be responsible for providing support to practices to ensure that patients receive regular check ups, medication reviews and Care plans.

The care coordinator will ensure that the persons needs are met, will value the important of their choice and decision making in their life, and will actively seek to encourage gifts and abilities.

The post holder will need to be motivated to develop the role based on the needs of the practices across the PCN, ideally creating a pathway to ensure all patients receive a high level of support to meet their health needs.

Person Specification

Experience

Essential
- Experience of working as a HCA
- Experience in a patient/customer facing role
- Administration experience

Desirable
- Experience of working in primary care
- Experience of working in a GP practice

**Qualifications**:
Essential
- GSCE grade A - C (or equivalent) in Maths and English, or higher level qualification

Desirable
- NVQ3 in Health & Social Care

Skills & Abilities

Essential
- Access to a vehicle and a full, clean driving licence is essential
- Excellent interpersonal and communication skills
- Excellent empathy and listening skills
- Prioritisation skills
- Good IT skills
