As a Community Navigator you will oversee our Independent Living site at Fitzwilliam Court in Barnsley that support customers over the age of 55. Working closely with our Independent Living and Health & Wellbeing teams, you'll develop and provide a customer focussed service that connects our customers to local services and their wider community to reduce social isolation. You’ll do this by fostering relationships with external organisations within the community to develop opportunities for volunteering, education/training, employment, companionship and health and wellbeing activities for our customers. You’ll collaborate with customers to coproduce a personalised Community Navigation Package based on their priorities, interests, values, cultural and religious/faith needs and motivations - and empower customers to code sign, run their own events and activities.

We’re looking for someone with energy and resilience who will work hard to develop the community connections. You will be creative and have ideas as well as enthusiasm to ensure our customers engage. You will also have.
- Commitment to, and a good understanding of, services that enable customers to live as independently as possible.
- Good communication skills including written and verbal.
- Experience of engaging and motivating people.
- A good level of IT literacy.
- Good multitasking and organisational abilities.
- Ability to lone work and work ‘on the go’ using mobile technology.
- A Full UK Driving Licence and access to own vehicle (business mileage paid)
